Ten quick questions about your desk, chair and screen. About 2 minutes. No sign-up; your answers are not stored.

Sit at your normal workstation and answer Yes or No.

Do your feet rest flat on the floor (or a footrest) with knees at about 90 degrees?
Does your chair support your lower back?
Are your shoulders relaxed (not shrugged) when your arms rest?
Is the top of your screen at or just below eye level, about an arm's length away?
Can you see the screen without turning or bending your neck?
Is your mouse right next to your keyboard, at the same height?
Are your wrists straight (not bent up or sideways) when you type?
Do you avoid holding the phone between your ear and shoulder?
Do you stand up or move at least every 30 to 60 minutes?
Hours a day at a computer
Do you have pain in your neck, shoulders, back, wrists or eyes after work?
